    • A deposit is required to hold your appointment. A Deposit Instructions Guidelines will be sent after appointment is made. IF DEPOSIT NOT RECEIVED WITHIN 24hrs after receiving the form, appointment will be canceled.

    • If there are no follow-ups or properly following the forms instructions in the electronic forms, the appointment will be canceled.
